Telephone/Voice Mail Request Form



Request for new or replacement phones (requires approval from manager), new extension numbers, reassignment of a phone, new voice mail or phantom voice mail box must be place through the Service Desk and will require two weeks notice.

A coverage path is the route an unanswered call will take after a specified number of rings. By default, voice mail is the first point of a coverage path after 3 rings.

Covering Extension
A covering extension is the phone extension where an unanswered call will transfer to if the caller presses the 0 (zero) button while listening to the voice mail greeting.

I understand that I am responsible for my usage of this phone extension and voice mail account. If I leave MCC, my supervisor or I will contact the MCC Service Desk at x3301 to have my extension and voice mail cancelled. I also understand that voice mail messages are automatically deleted from the system after seven days.

Service Level Agreement: All requests for assistance are considered important and the Technology Center staff will do their best to resolve all reported problems in a timely fashion based upon impact and urgency within the constraint of staffing levels and incident and request volume. Due to the volume of incidents and requests received during the opening two weeks of the Fall and Spring Semester, during finals, and other peak volume times, response times may be longer than average. Response time can also vary depending on the volume of requests at any one time and the number of staff on duty. The technical staff will inform customers if such exceptions are necessary.

The Technology Center reserves the right to prioritize service requests based on the number of people affected and severity of the issue.
