MEDICAL HISTORY, IMMUNIZATION FORM AND HEALTH INSURANCE:
All International Students are required to submit the completed International Student Health Record and must purchase the Middlesex Student Insurance.

Health Program applicants:

Admission to a Health Division program is selective because of limited class size and clinical sites. Students considering applying to a Health degree or certificate program with additional admission requirements should thoroughly review prerequisites and other conditions before applying to that program. Non-Massachusetts residents, including International students, will be admitted on a space available basis only. Individuals who lack the prerequisite courses for a particular health program may enter the college in the Liberal Studies with Health Pathway program and, once prerequisites have been completed, they may apply to be considered for admission to a health program.
