This form is required for all fundraising efforts organized by MCC clubs and organizations.

Terms and Conditions - Please read carefully:
1. This form must be completed in its entirety and submitted no later than three weeks prior to fundraising activity.
2. All funds will be deposited into your club account and distinguished as "revenue".
3. Deposits must be made daily, therefore students should continuously drop their profits off to the Student Accounts Office or Student Activities Office.
