This form is required when requesting funds for MCC clubs or organizations, student programs or events. All clubs requesting funding must have a current copy of their constitution on file in the Student Activities Office.

Terms and Conditions:
1. Under most circumstances, review of this request will take place no sooner than two weeks after submission. It is recommended that a student be present to defend this request and answer questions.

2. Clubs and organizations may not spend funds until a budget has been approved.

3. Please submit any supporting documents to the Student Activities office or via email to Melissa Welch or Kim Clark.
