Financial Aid Forms

As part of the financial aid application process, some students will be required to submit documents in addition to the Free Application for Federal Student Aid (FAFSA).

The Financial Aid Office notifies students if additional documents are needed for their file by sending the student a Missing Information Letter. Each month these letters are mailed and/or emailed to the student's home address and/or MCC email address on file with the Student Information Center. Students may also see if any additional documents are needed by logging into Middlenet.

In 2019-2020 our forms will be completed online. You will be notified via email if you will need to use our online process.
