At least two weeks prior to the start your first summer camp, please mail, e-mail or fax to the address below.  In addition, you must include an updated immunization form signed by your child's doctor.   NOTE:  These forms only need to be filled out ONCE for the entire summer program, even if attending multiple weeks of camp.
